Secondary Premature Ovarian Failure
Restoring Life and Hope: A Case Study in Secondary Premature Ovarian Failure
By Rebecca Provorse, ND
As a physician, I encounter patients facing unique and challenging health journeys, and each one reminds me of the transformative power of physiologic hormone restoration. One particularly compelling case involved a young woman who had endured a series of medical battles and was struggling with the long-term consequences of her treatments. Her story is one that underscores the importance of addressing hormone health as a foundation for overall well-being.
A Life Interrupted
At just 20 years old, my patient arrived in my office overwhelmed by symptoms that no one her age should have to endure—insomnia, anxiety, depression, hyperlipidemia, and osteopenia. Her health history was complex: diagnosed with acute myeloid leukemia (AML) at age nine, she had undergone intense rounds of chemotherapy, radiation, and even surgery. The cancer recurred a few years later, requiring another wave of aggressive treatments. While she had begun puberty, her development was never allowed to fully progress, halting at Tanner stage four.
By the time she reached my office, she was in her third year of college, trying to keep up with her studies while managing a regimen of statins, antidepressants, and oral contraceptives—prescribed primarily to induce artificial cycles and slow bone loss. Despite these medications, she was still exhausted, unfocused, and emotionally drained. Her body wasn’t functioning as it should, and she was struggling to move forward in life.
A New Approach: Rhythmic HRT
After a thorough evaluation and discussions about her treatment options, we decided to start her on Physiologic Hormone Restoration Therapy (PHRT)®. Unlike conventional hormone replacement approaches, PHRT® is designed to mimic the body’s natural hormonal rhythms, promoting balance and long-term health benefits.
She applied her prescribed estradiol and progesterone creams twice daily, and after just three months, we saw remarkable progress. Her insomnia was improving, her energy levels were rising, and her vasomotor symptoms—hot flashes and night sweats—had disappeared. She was finally experiencing menstrual cycles, though they were still shorter than ideal. Adjusting her dosing allowed us to extend her cycle length, bringing her even closer to a natural hormonal pattern.
Transformation Over Time
Over the next year, her progress continued. With improved hormonal balance, her lipid levels normalized, allowing her to discontinue her statin medication. Her mental health stabilized, and by the second year of treatment, she no longer required antidepressants or anti-anxiety medications.
One of the most exciting milestones came when we added testosterone to her protocol, further supporting her physical and emotional well-being. As a result, she gained muscle tone, improved focus, and a greater sense of confidence. A follow-up bone density scan revealed that she had nearly reversed her osteopenia—a profound testament to the power of proper hormonal support.
Perhaps most meaningful of all, she was finally able to fully mature—both physically and emotionally. Her secondary sex characteristics completed their development, and for the first time in her life, she felt comfortable enough in her own body to begin dating.
The Bigger Picture
This case is a perfect example of why restoring hormones in a physiologic manner is so crucial. Too often, patients—especially young women—are dismissed with band-aid solutions like birth control pills, statins, or antidepressants that fail to address the underlying cause of their distress. By recognizing the critical role of hormones in metabolism, mood, bone health, and overall vitality, we can provide real solutions that change lives.
For my patient, this wasn’t just about symptom relief—it was about reclaiming her future. And for me, it was another affirmation of why I do what I do: helping patients restore, rebalance, and thrive.
